Supervised Visitation Order in Divorce?

I am currently in the middle of filing for a divorce in the state of Texas. My soon to be ex husband is in jail currently for his second felony, this one being possession of cocaine. I am filing for supervised visitation under my supervision for our 3 year old daughter. I am doing this divorce on my own with no help from a lawyer due to monetary reasons. I have done everything in this case and am up to point of going to court with the final decree... but heres where my problem sets in... i am trying to figure out how to word properly that my exhusband and I both agree upon supervised visitation. I dont know how to phrase it in writing for the judge and what i should expect as well with doing it this way. any advice is appreciated.. thank you
